Dialog Faculty/Staff Submission Form

UA faculty and staff may use this form to submit items for Faculty & Staff Notes to run periodically in Dialog. Submitted items should deal with activities that have occurred during the current academic year. FS Notes cites published works, performances, appointments, awards and elected positions. Dialog regrets being unable to print information on presentations, speeches or works yet to be published.

In order for Dialog to maximize the layout of FS Notes, the University Relations staff may request copies of publications or covers for scanning, a recent staff photo or other material. Please call 348-8332 to update a file photo.
