Benefits Open Enrollment Offered through Nov. 27
The open enrollment window for benefits will continue through Nov. 27. Open enrollment provides employees, retirees and COBRA participants a chance to enroll in new coverage or change or cancel existing coverage. It applies to the health, dental and vision plans and to the flexible spending account plan known as BAMAFLEX. All UA employees should have received a packet of information in their mailboxes. Anyone who has not received the packet should contact the Human Resources Service Center at 348-7732 as soon as possible. Participants may enroll for benefits by downloading forms from the Benefits Web page at http://bama.ua.edu/~hr/, by picking up forms in the HR Service Center (G-69 Rose Administration Building), or by calling 348-7732 to request forms to be sent via campus mail. All enrollment and change forms must be returned to the HR Service Center by 5 p.m. on Wednesday, Nov. 27. Forms may returned in person to G-69 Rose Administration Building or mailed to Box 870174. Faxed copies cannot be accepted. If you do not enroll or make changes to your medical, dental or vision plan elections for 2003, you will automatically continue your current benefit elections in 2003. The exception is your BAMAFLEX account. Even if you are presently enrolled in BAMAFLEX, you must complete a new election form to continue in 2003. If you do not complete a new BAMAFLEX election form, your BAMAFLEX participation will end Dec. 31, 2002. See Personnel Matters for more information. |
